Sports Physicals

In North Carolina all student- athletes must have completed a sports physical exam to participate in practices and athletic contests. 

 

What Is a Sports Physical?

A sports physical — also called a Pre-Participation Physical Examination (PPE) — is required by most North Carolina schools and athletic programs before a student athlete can take the field. At Western Wake Wellness, we make it simple to get your child evaluated and cleared so they don’t miss a day of practice.

Why have a Sports Physical?

A sports physical is a focused health evaluation to determine whether your child is safe to participate in organized athletics. It is not a replacement for an annual well-child visit, but it is specifically designed to identify any conditions — known or unknown — that could put a young athlete at risk during strenuous physical activity.

      What the Exam Includes

      Our sports physicals cover:

      Medical and family history review, including past injuries, surgeries, chronic conditions, and any family history of heart problems

      • Vital signs: height, weight, blood pressure, and resting heart rate
      • Vision screening
      • Heart and lung evaluation (listening for murmurs or irregular rhythms)
      • Abdominal exam
      • Musculoskeletal assessment: joint range of motion, strength, and flexibility
      • Review of any current medications, supplements, or ongoing concerns

      Who Needs One?

      Any student athlete participating in school-sponsored or recreational sports should have a current sports physical on file. The NC High School Athletic Association (NCHSAA) requires a completed physical examination form before each new sports season. We recommend scheduling 4–6 weeks before the season begins so there is time to follow up on any findings before the first practice.

        We use the standard NCHSAA Pre-Participation Physical Evaluation form. You can download the current form directly from the NCHSAA website, or the Wake County form here, or we can provide a copy at your visit. Please bring a completed medical history section if possible — it helps us make the most of your appointment time.
